Une rumeur étrange circule dans toutes les écoles du Japon. Dans le vieux bâtiment, une certaine dame prénommée Hanako se trouverait dans la 3e cabine des toilettes des filles du 3e étage. Ceux qui l’invoqueraient verraient un de leurs souhaits se réaliser. Nene Yashiro est une jeune fille fan de superstitions et d’affaires occultes. Elle aimerait bien sortir avec un camarade plus vieux pour qui elle a un faible. Elle décide alors de voir si la légende dit vrai et invoque Hanako. Mais une malédiction s’abat sur elle, et elle se retrouve transformée en poisson.
