Make a exercise plan that suits you. Start small and work your way up.

Read more. 5 pages is enough, just make it a habit since you cant avoid reading your entire life. Get better at it maybe even enjoy it.
It's alright to take rest days. avoid the mindset of all or nothing, 1 day isn't bad. Acknowledge if you are sick and focus on resting.
Psycho analyse yourself (get a therapist/psychiatrists if needed)
